Gut Health Check: A Simple Reflection Tool
The Gut Health Check is conceptually informed by the six-domain framework outlined by the International Scientific Association for Probiotics and Prebiotics (ISAPP), which recognises gut health as a multidimensional construct encompassing: 1) digestive function, 2) microbiome activity, 3) barrier integrity, 4) immune function, 5) metabolism, and the 6) gut–brain axis.1,2
However, as many of these domains require specialised or not yet widely validated measures, this tool is intentionally limited to a consumer-friendly reflection of aspects that can be captured through self-report—primarily digestive function (including motility, informed by the Bristol Stool Chart), gut–brain axis interactions, and key lifestyle factors such as sleep, physical activity, and dietary diversity.
In clinical practice, gut health assessment relies on patient-reported symptoms to distinguish normal variation from clinically relevant concerns, with investigations used when appropriate for further clarification. Symptom impact on daily life and quality of life often carries greater weight than biomarker-based measures.1 In this context, the Gut Health Check is designed as a structured, non-diagnostic reflection tool to support awareness and discussion, providing a simple score, plain-language interpretation, and targeted suggestions rather than a comprehensive assessment.

Scope and limitations
This tool is designed for general reflection only. It does not diagnose gastrointestinal conditions, replace validated clinical tools (e.g. IBS-SSS, GSRS), assess disease activity or severity, and include objective physiological or biomarker data. Given the complexity of gut health and the lack of a single standardised metric, comprehensive assessment should integrate validated symptom tools, clinical history, and (where relevant) objective testing.1

- Pre-consult: Patients complete the tool to reflect on symptoms and habits
- During consult: Use responses to guide discussion and identify key focus areas
- Education: Reinforce the multifactorial nature of gut health
